Physical Therapist - Out Patient Rehab at Home Medicare Part A
COMPENSATION: $95,000 - $120,000
Locations: Greenwich, CT & Westport, CT
Boutique home health agency for Seniors and recently hospitalized patients undergoing rehabilitation often require in-home physical therapy to increase strength, regain range of motion, and promote stronger emotional health and wellbeing.
We offer In-Home Physical Therapy Services for homebound older adults with a skilled need under Medicare Parts A and B, as an outpatient rehabilitation at-home service under Medicare Part B, and as a private duty service.
Responsibilities
Patient Base: Seniors - Patients are homebound with a skilled need (skilled need example-patient breaks their hip, has surgery and receives therapy at home to rehab the hip) -Post Acute
Needs: “1” Medicare Part A therapists
Shifts: M-F 8:30a-5p (hours are flexible as long as you see your weekly required visits)
Weekly Visits: Part A therapist is 27 visit equivalents a week /starts of care = 2 equivalents– a regular visit = 1 equivalent
Full time position
Med A is a higher-level PT
Part A has more documentation then Medicare Part B
Qualifications
Education: DRPH or someone with Homecare experience (DRPH preferred not required)
5+ years of experience and home health experience preferred
BLS and ACLS
National Institutes of Health (NIH)
Sound knowledge of anatomy, physiology, pathology, and medical terminology.
The ability to stand for extended periods.
Strong technical skills.
Knowledge of medical terminology
Passionate about patient care
Requires a minimum of two years of current Physical Therapist experience.
Graduate from a fully accredited Physical Therapist program
Active unencumbered Tennessee Physical Therapist state license
One year of EMR experience
Requirements
