Cooking Classes, Chair Yoga, Silver Sneakers, Senior Prom’s, Birthday parties, and other events to engage our Senior Patients and keep them active and connected.
Responsibilities
Provide care to Seniors 65+ years old. Physicians practice at the top of their licenses, complete charting during office hours, and have a life outside of the office. They have the freedom to provide the kind of care that you would want your grandparents to receive. Our goal is to provide a concierge level of service for Seniors who do not have access nor can afford such a high level of care.
• Primary Care – outpatient
• Value Based Care model.
• Shifts: Monday – Friday 8a-5p
• Call: 1:8 Weeks. Average week 3-5 total calls 1 week of call every 8 weeks.
• Patient panel max of 400
• Average patients per day: 10-13. Starting at 6 patients per day and working up
• Lots of initial training
• Patient satisfaction scores are usually a 5
• Ea. provider has their own care team / schedular, MA, referral specialist, Nurse, Case Manager, Social Worker and Outreach Specialist
• EKG’s and Labs in the Center
• Have cooking classes, yoga, silver sneakers-mall walking exercise community for seniors, social community health coordinator

Qualifications
Board Certified/Board Eligible in: FM, IM or Geriatric Medicine
Open to Residents and Fellows
Open to Visa Candidates
Basic Life Support (BLS) Certification
Advanced Cardiovascular Life Support (ACLS)
National Institutes of Health (NIH)
Sound knowledge of anatomy, physiology, pathology, and medical terminology.
Active unencumbered physician Alabama state license
Graduate from a fully accredited MD or DO program
One year of work experience as Family Practice Physician
One year of EMR experience
Board Certified

Participates in relevant continuing education programs.
Knowledge of and dedication to the highest patient care and provide the utmost service excellence to all patients.
Ability to take initiative and to exercise independent judgment, decision-making and problem-solving expertise.
A can-do, team oriented attitude is essential, as the ability to work independently and to interact easily with physicians, other managers and staff throughout the department and hospital.
Effective interpersonal/communication, organizational, and the flexibility to handle multiple tasks.

• Base Salary: $250k for a resident. More for experienced PCP with Value-Based Care Experience $280k/ Experienced PCP without Value-Based Care experience $26k5-$270k
• Sign on: Up to $15k.
• Annual incentive bonus: up to $28% of base pay (most make around $20%)
• Partnership after 3 years (no buy-in)
