Job Details: - Working in the town of Edison, New Jersey
- 15-20 patients per day
- No-cap incentive compensation plan
Responsibilities: - Drive high-quality, safe, and cost-effective patient care in post-acute care communities
- Collaborate with other clinician interdisciplinary teams and follow CMS guidelines
- Work closely with hospice, palliative care, and home care agencies for safe patient transitions
- Ensure timely documentation and billing through our EMR (Gehrimed) meeting Medicare guidelines

Competitive compensation, with an estimated salary range of $161,000 to $260,000 annually with no cap productivity income potential.
