Responsibilities: - Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care in a skilled nursing facility
- Implement an individualized plan of care for patients' goals and provide clear communication with patients, clinicians, facility leaders and staff regarding care and services for enrollees
- Work alongside pharmacists on medication management
- Complete acute, follow up, discharge, and wellness visits as necessary
- Make recommendations on transitional care, DME, home health services, or outpatient services after short stay
- Timely response in chart completions and pharmacy recommendations
- Provide oversite to TeamHealth clinicians providing coverage in the same facility
Experience as a medical director in a skilled nursing facility preferred. However, clinicians trained or practicing medicine in the following disciplines are encouraged to apply: family (FM), geriatric, primary, emergency (EM), internal (IM), adult, post-acute, home health, hospice, palliative, and correctional medicine (HM).
Expected compensation is estimated range of $50,000 to $77,00 annually with no cap on productivity income potential.
