Responsibilities Include:- Enhance cognitive, psychosocial, and vocational interventions that empowers patients receiving physical rehabilitative care to achieve an optimal level of overall functioning
- Cultivate independence and personally fulfilling, socially-emotionally meaningful, and functional effective interactions
- Establish patient goals to facilitate independence, social, and psychological adjustment
- Conduct clinical case reviews, share tools, information, and guidelines as they relate to quality of care
- Make recommendations on transitional care or outpatient services after short stay
- Develop intervention plans, and share ideas about network management issue
- Facilitate and assist leadership in quality improvement initiatives and risk management
- Represent TeamHealth in engaging, establishing, and maintaining relationships within the community
- Provide clear communication with clinicians, facility leaders and staff regarding care and services for enrollees
- Timely response in chart completions

Expected compensation is estimated salary range of $100,000 to $140,000 annually with no cap on productivity income potential.
